Contract/per diem Nurse Practitioners needed for in home assessments of Medicare patients. Paid per visit, all expenses paid. Full time or Part time hours are available including self scheduling options. Each Nurse Practitioner will be responsible to complete a medical assessment in a home environment. Assessment shall mean a medical examination to obtain a comprehensive medical assessment of Members (Patient) for the purpose of facilitating the Program's disease management, care management, case management and medical therapy management services. The qualified Practitioner will ensure the patients within this program have a clinical evaluation that can be used in communicating with the Member's physicians concerning the Member's medical condition and their personal medical needs documented in a way that captures the Member's condition in a timely, accurate and complete way.

    Minimum Skills, Knowledge and Ability Requirements

    • Minimum 50%, up to 75% domestic and international travel required. Flexible working hours are required including nights and possible weekends. Office based when not traveling.
    • Pharmacy technician or experience in hospital pharmacy environment highly desired.
    • Good analytical mind that enjoys working with software and technology.
    • Strong customer service, interpersonal, and training skills. Must be customer focused and have experience managing customer expectations and facilitating problem resolution.
    • Excellent oral and written communication skills.
    • Detail-oriented with ability to multi-task and manage different projects.

    Job Duties:

    • Perform Centers for Medicare and Medicaid Services (CMS) required Medication Regimen Reviews as required by law
    • Identify patients receiving medication known to have high potential of risk and manages associated risk by improving monitoring, discontinuation, or change to a safer alternative medication if one exists or to implement non-pharmacological medication interventions
    • Serve as liaison between the contracted provider pharmacy and living centers to monitor for, act on, and resolve any pharmacy related issue that may adversely affect residents
    • Identify strategic areas for improvement in order to effectively increase ability to accomplish clinical activities off-site
    • Collaborate with other stakeholders in the development and delivery of training for multiple facility personnel

    Qualifications:

    • Registered Pharmacist Licensure in appropriate state(s)
    • Doctorate of Clinical Pharmacy (Pharm.D), preferred
    • Board Certification as Geriatric Pharmacist (CGP) required or must be obtained within first two years of employment
    • 3-4 years experience as consultant pharmacist in long-term care living centers, preferred
    • Managerial experience in geriatric pharmacy, preferred
    • Must be able to travel with overnight stays

    RESIDENCY OPPORTUNITIES

    IHS residencies offer an excellent opportunity to learn how to be a pharmacist while serving in a progressive practice setting helping to improve the healthcare of Native Americans.     STUDENT OPPORTUNITIES

    The IHS offers three excellent opportunities for students throughout the academic year through the Junior Commissioned Officer Student Training and Extern Program (JRCOSTEP), Senior Commissioned Officer Student Training and Extern Program (SRCOSTEP) and Senior Year Externship Rotations. All of these programs are highly competitive.

    BENEFITS

    Commissioned Corps benefits are equivalent to those of the other uniformed services, including the military services. Unlike the military, however, the Commissioned Corps allows officers to separate from the service at any time of their choosing, or to remain at their current duty station until they choose to relocate or separate. In other words, the officers decide for themselves when and where they will move.

    Officers are encouraged to remain in the service for at least two years. Upon so doing, they earn two additional benefits: Unused annual leave assumes a cash value upon their separation, and the officer's relocation home or equivalent destination is covered. Officers leaving prior to having served at least two years relinquish these two additional benefits.

    Another benefit of the Commissioned Corps is the officer's discretion in selecting his or her assignment. A commissioned corps applicant is not called to duty until and unless a favorable job match is achieved. Geographic preference is therefore given primary consideration when attempting to find suitable assignments for the applicant. Once a position is identified that appeals to the applicant, he or she may then accept that specific assignment and establish the call-to-duty date. If no opportunities appeal to the applicant, then no assignment is made. That is, there is no obligation whatsoever in applying to, and qualifying for, the Commissioned Corps. The application is good for a full year; if no match is made or the applicant finds employment elsewhere, the paperwork is simply withdrawn.

    PAY

    The pay and benefits of the PHS Commissioned Corps are equivalent to those of the other uniformed services, e.g., the U.S. Army or the U.S. Navy. All newly graduated pharmacists with a Doctor of Pharmacy degree are commissioned at the O-3 pay grade, which is the same rank as an Army Captain or a Navy Lieutenant. The USPHS uses the same designations as the Navy; thus, newly commissioned Pharmacist Officers enter at the Lieutenant rank. The basic pay is established according to that pay grade. With an increase in rank (eligibility for promotion occurring just 4 years after graduation), the basic pay increases substantially. Pay also increases over time, starting with a pay raise on one's second anniversary, another on the third anniversary, and again on the fourth anniversary. Additional pay raises generally occur every other year thereafter. Cost-of-living adjustments further increase the basic pay.
